What the form collects
Enough to enroll a child and to look after them once they start.
Student — name, preferred name, date of birth, gender, home address, grade or program
Parents and guardians — names, relationship to the student, email and phone numbers
Emergency contacts — two people other than the parents, with two numbers each
Authorized pick-up — who else may collect the student
Health — doctor, health card number, allergies, Epi-Pen, ongoing conditions, medication taken during the day, dietary requirements
Permissions — photographs, local outings, sunscreen, emergency medical treatment
Billing — who is invoiced, whether fees are split between two parents, subsidy, siblings enrolled, payment frequency
Declaration — a parent's signature and the date
Using the form
It prints as a single double-sided sheet. Many schools send it out with the family welcome package and keep the signed copy in the student’s file.
